Female Athletes, Hormones & Injury Risk: Why Energy Availability Matters More Than You Think

Female physiology introduces additional layers of complexity in injury risk and recovery — particularly through the interaction between energy availability, hormonal status, and connective tissue integrity.

This is highly relevant for recreational exercisers, not just elite athletes.


The Hormonal Link to Injury Risk

Oestrogen plays a key role in:

  • Collagen metabolism

  • Bone mineral density

  • Ligament laxity and stability

  • Muscle repair signalling

Fluctuations or chronic suppression of oestrogen can therefore influence injury susceptibility.



Low Energy Availability in female exercisers

Women are particularly vulnerable to Low Energy Availability due to:

  • Intentional dieting alongside training

  • High activity + under-fuelling patterns

  • Misinterpretation of “clean eating”

  • Training for body composition goals

This can lead to:

  • Menstrual disruption

  • Reduced bone density

  • Impaired recovery capacity

  • Increased stress fracture risk

This cluster sits under the broader umbrella of Relative Energy Deficiency in Sport (RED-S).


Tendons, ligaments, and hormonal influence

Connective tissue is sensitive to hormonal environment:

  • Oestrogen influences collagen turnover

  • Low energy availability impairs synthesis rates

  • Recovery from tendon injury can be prolonged

This helps explain why some women experience:

  • Recurring tendon issues

  • Slower rehab progress

  • Increased soft tissue sensitivity during high training stress phases


Perimenopause and changing recovery capacity

In midlife female athletes, additional considerations include:

  • Declining oestrogen levels

  • Changes in muscle protein synthesis efficiency

  • Reduced connective tissue resilience

  • Shifts in fat distribution and energy utilisation

This does not reduce capacity to train — but it does change recovery demands.


Practical nutritional priorities for female athletes

  • Avoid chronic under-fuelling

  • Ensure adequate protein intake across the day

  • Support calcium and vitamin D status

  • Match carbohydrate intake to training demand

  • Prioritise recovery nutrition, not just performance nutrition
